From my own personal experience a 'Deepest Sympathy' card or a heartfelt letter of condolence really means a lot too...

Ditto that.

From the giving end, I have always been very careful of what I say and tried to make it appropriately short, according to the person I was addressing.

From the receiving end, while I "heard" the person speaking to me, I didn't hear anything they said but it was important to me they came and offered their support. Heartfelt hugs meant a lot, as well.
