There is no way this was one isolated incident 5 years ago. More likely it was ongoing for years & the employee brought it to management's attention repeatedly.
People who have worked & lived a normal life for many years don't just wake up one morning & say, "I think I'll kill 5 people today."
I've worked in places where such harassment is permitted - even encouraged - where supervisors & management are part of it, so they do nothing about it. It goes much further than ignorant racists leaving nooses or making comments; I had a supervisor (who I turned down for a date) actually sabotage my work, then write me up & give me negative job performance reviews, which resulted in reprimands, no raises or promotions & (in some cases) termination.
And, after an employee snaps, the company is only interested in protecting their image, so they pretend to be shocked at the employee's "sudden, irrational behavior."
