I developed this form to help the next of kin in case of especially an unexpected death. Having this information filled out and stored in a place that you can access it quickly (copy on the phone too) may really help.

That's sweet of you to offer that. I have already, hubby and I filled out forms like this and made copies for our immediate
family so they have it on hand when needed. Our DNR is notarized 2 copies of each of us and closest son has one set for each of us.
So we have that all done already. Once again, how very nice of you to offer for others just in case.

We have forms filled out for this already but thanks for bringing it up. It is a very important thing to fill out.
