Info Required When You Got Your Vaccination

When you got your vaccine, what kind of identification did you have to show?

In Canada, at least in BC, we registered ahead of time based on age or other criteria. We showed our provincial medical card & DL or similar ID. This was recorded along with a code for the type of vaccine we were given. Essentially it’s all in a data base. We were given a proof of vaccine card. Should we want to travel to another country that requires it, it could be verified.

The reason I’m asking is when our borders open to the US on August 9th, basically only fully vaccinated will be allowed in. There’s an online registration that must be filled out prior to arriving at the border. That’s about all we’ve been told.

The US isn’t opening their borders yet.

The nurse asked for my name and dob. I gave her my drivers license and insurance card. She checked my name on the drivers license and did not need the Medicare card. She filled out my vaccine card and was on her way.

I was only asked my name and DOB. After the second shot, the nurse signed the vaccination card that they gave me when I got the first shot. My doctor's office set up the appointment (at their remote vaccine station), so all my info was already in their system.

At the place of vaccination, all they asked me was my name and DOB I had already registered online with our Department of Health to get the vaccinations when my turn came, so they already had my info when they called me with the appointments. My sister got her vaccinations at her regular health care place, so they already had her information.
