This was posted on my patient portal. I had to ask someone that knows about these things what it meant. Doctor's statement in red - interpretation in black, for what it's worth:
S/p SiO exchange/PPV/Mx ILM and SR bands/Retinectomy/EL OS 11/1/22 (JW/NF):
S/p: Status post, meaning the patient underwent the following procedures in the past.
SiO exchange: Likely refers to the exchange of silicone oil, a substance used to keep the retinain place after certain retinal surgeries.
PPV: Pars Plana Vitrectomy, a surgical procedure involving the removal or replacement of the vitreous gel in the eye.
Mx ILM: Peeling of the Inner Limiting Membrane, often done to treat certain retinal conditions.
SR bands: Subretinal bands,which might have been removed.
Retinectomy: A surgical procedure involving the removal of a portion of the retina.
EL OS 11/1/22: Eye surgery on the left eye (OS) on November 1, 2022.
(JW/NF): Possibly surgeon's initials or other identifying markers.
Amniotic graft aborted due to extensive PVR RRD:
An amniotic graft was planned but not performed due to severe proliferative vitreoretinopathy (PVR) rhegmatogenous retinal detachment (RRD).
Posterior stretch hole found at the time of surgery with associated SR bands extending into the periphery which were removed at the time ofsurgery:
A hole in the posterior part of the eye was discovered during surgery, with subretinal bands extending toward the edges of the retina. These bands were removed during the surgery.
Retinaflat with good SiO fill; IOP 9 OS today:
The retina is flat after the surgery, with satisfactory silicone oil (SiO) filling. Intraocular pressure (IOP) is 9 mmHg in the left eye today.
FTMHopen with scant SRF:
A full-thickness macular hole(FTMH) is open, with limited subretinal fluid (SRF).
Significant retinal atrophy and loss of both IS-OS as well as NFL loss:
There is notable retinal tissue degeneration, loss of the inner and outer segments of the photoreceptor layer, and loss of nerve fiber layer (NFL).
No plans for SiO removal:
There are no immediate plansto remove the silicone oil.
Pt started using AT BID OS. Recommend increasing frequency to QID:
The patient began using "AT"(likely artificial tears) twice daily (BID) in the left eye. The recommendation is to increase the frequency to four times daily(QID).
PVD OD- Pt notes new floaters OD:
The patient has a posterior vitreous detachment (PVD) in the right eye, and the patient has reported experiencing new floaters in that eye.
Noted some paving stone degeneration superotemporal and inferior OD:
The surgeon observed an eye condition called "paving stone degeneration" in the upper temporal and lower areas of the right eye.
RT/RDon 360 SD exam. RT/RD precautions reviewed. Monitor:
The surgeon likely performed a 360-degree scleral depression (SD) exam and identified retinal tear(RT) or retinal detachment (RD). Precautions for RT/RD were discussed and monitoring is recommended.
PC/IOLOU s/p YAG OS - st:
The patient has posterior capsule (PC) and intraocular lens (IOL) after undergoing YAG lasercapsulotomy (a procedure to improve vision after cataract surgery) inthe left eye (OS). "OU" stands for both eyes. "St"is unclear without more context.
