The World Doping Agency's data base was hacked and testing/medical records leaked on several athletes including US athletes. Gymnast Simon Biles was found to have a therapeutic exemptions for ADHD drugs and from tennis the Williams sisters were found to have exemptions for pain killers, anti inflammatory steroids and asthma medicine.

What's wrong with athletes having an exemption for prescribed drugs?

Because the drugs in question by themselves in a 'normal' person act as a performance enhancing drug. The question is do athletes really have those conditions. For years racing athletes like runners & swimmers were known have taken asthma medication to increase lung capacity. Better lung capacity means better endurance. Makes it easier to wear down an opponent in a sport like tennis. ADHD medications act like speed in a normal person. Also can give a mental edge of sorts by increasing alertness. Some say Biles could have benefited by quicker reaction times and timing on movements along with the 'energy' to workout and practice longer.
