Dentist's office didn't know my late husband???

I found an appointment card recently for my late husband to have his teeth cleaned in March. He died in October, so I called to cancel that appointment. She said she didn't have anything scheduled for him on that date and said she didn't have him in their computer. What??? He's been going there since we moved here in 2022. He had 2 crowns (one put in 8 days before he died), a filling, and 2 cleanings at their office. She asked me his birthdate and said...nope, don't have him.

I was dumbfounded. I couldn't figure it out. Have you ever had anything like that happen to you?
 
My existence has been lost online a couple of times over the years, but never in "real life", no. Is it a possibility that he may have cancelled that appointment after they filled out the appointment card? Maybe requested to no longer be a patient at that practice? That's all I can think of that would make at least partial sense... if he removed himself as a patient/client, he would possibly no longer be in their records.
 
The dentist's office may have been notified (digitally) by the Social Security Admin.

My mother died in 2021. Her bank wasn't notified by SSA until a whole year later, which created challenges for us.

Apparently things have improved; SSA's system was upgraded.

No, I always took care of his appointments and I certainly didn't cancel it before calling yesterday. They should have at least had his name in their computer, which they didn't. Very strange.

I don't think SS would have called a dentist's office.

So, the card is something you have in a file?
The dentist's office may have been notified (digitally) by the Social Security Admin.
Or some other entity, and they deleted his acct.
It's most likely in their hard copy files (I think they have to keep a file for a certain amount of time)
Seems whoever you talked to didn't make the effort to check those files

Happened to me when I requested a history of an eye exam from five years ago
The lady said 'we don't go back that far'
Then later found them in a hard copy file cabinet
