The form is here, but there is a catch during this pandemic...

It states that it has to be signed by a doctor. Not a reasonable option now. I'll try to reach out. My current DNR is with my prior physician. I am not going to bother my current practitioner with it. Perhaps I can contact my state health department about filing these online. As I mentioned in question about ventilator priorities, I really think this is something we must do to protect younger generations.

I have a DNR and my current doctor gave me the forms. I filed with him and both daughters have copies. As far as the vent is concerned. I posted elsewhere that one saved my life at age 50.At my current age, 72, just let me go. Save my vent for a younger person.

Totally agree. Can't imagine being the first to need and get a ventilator and then see a younger person come in with the same need and be deprived. It would not make sense, in my opinion.
I did email my Ohio Dept of Health and place a call to the coronavirus hotline about DNR's not being valid unless signed by a doctor. I just don't think that's reasonable now - they have far more important things to do! I don't see why we can't do it online with E-signatures.

The main thing is that your family knows your wishes... AND POST YOUR DNR...
NC as a medic only the ORIGINAL form can legally accepted. I have had several cases were the family says they have a DNR but dont have access or know where it is...
My grandfather collapsed at home, His niece started CPR not knowing he had a DNR.. Grandmother did but in the moment she was lost
1st responders arrived and worked him, got him back, so he lived brain dead on a vent for 3 days... ripe old age of 96.

This is exactly what I don't want to happen to me - Prolonging the death process! My daughter has my old DNR which pretty much reads like your lovely MOST form, but much wordier and less organized. My prior physician also has/had a copy. Thanks for your help. Much appreciated! https://www.ncmedsoc.org/non_members/public_resources/MOSTform_sample.pdf

The reason for the new DNR rules according to one doctor is recessitation takes about two minutes to start from the moment of alert. In that time the brain and tissue are impacted. Second, when they try to recessitate the body expels a lot of material from the throat and lungs patient.
