Errors in Medical Records

2018 was not a good year for me. I had a medical problem. When I go to the ER, now, I can say that I was in that room, and that one, and that one, and that one. Well that one is my favorite. But I have a jumble of electronic medical files. I have allergies listed that I don't have, misdiagnoses, wrong medications, and wrong dosages. You'd think it would be easy to edit out the misinformation, but it isn't. I have tried repeatedly. My doctor has tried repeatedly. We have both been told it is impossible. Even if we could erase the errors, some computer is going to re-enter them. So, if when I see a health provider, I have to make sure they have the correct info.
 

when i go to the doctor i have all my medication written down on a index card--so i will know what i am getting --she has asked me if i was still taking a certain medicine that i have never been on
 
You cannot leave anything up to your healthcare provider, doctors and staff. You either assume full responsibility for your own healthcare, or play Russian Roulette with those who honestly don't care if you live or die, as long as they get paid. Sad stuff, that.
 

Both you and I are from PA. I am a patient in the Geisinger medical system located in PA and have Geisinger Gold, their Medicare Advantage coverage. All my medical records are computerized for as long as I have been part of the system. I can do loads from MyGeisinger.org as a member. I can order or renew medications, get my blood test results, and contact my doctor or vice versa if necessary. When I walk in for a visit, all my info is at their fingertips. Check it out.

And now i hear on the news where CVS & Walgreens are going to go "more medical" . As in mini-clinic , perhaps two docs or PA's on staff, some lab services, etc. What will this do for our medical care? As the doctors take their shingles down, and retreat to hospital work only.
 
Earlier this year, Member GreenSky (Rick) was working with us to get my wife a lower Supplement Premium. The Insurance Company denied my wife's application due to her drug history!! My wife has taken 2 drugs a day for the past 5 years!!

Rick got the reporting agency to send him a copy of my wife's history. For the past 5 years, there where 95 entries from 2 pharmacies and 5 doctors. 46 entries and 3 doctors where not my wife's and where in a different state!!

Rick contacted the pharmacy and they researched their records and corrected the history record..

My wife was accepted insurance coverage at a savings of $170 a month!!
 
Wouldn't it be nice if something actually works right?
What was it we were told about the computerized, paperless office? You push the button and they do the rest (badly).
I think that's called GIGO.
 
About 5 years ago I had a colonoscopy when I was about to be given the sedative the Dr said you had a polyp removed last year I said NO I didn’t I hasn’t had a colonoscopy for 10 years
But the doctor insisted in was me despite my words she gave me the exact dates , the name of the hospital ,and the Drs name who’d preformed the colonoscopy

I keep a diary so after a couple of days I got my diary’s out to look up what I was doing on that date
I was actually at the Adelaide airport welcoming my new DIL who had lived in Australia for 6 months prior but had to go back to, the Philippines to get her permanent visa to live in Australia
so with that proof I contacted Medicare ,the hospital and pathology not one of then had any records of me having any medical procedures on that date
The dr apologised to me in writing saying a mans results had somehow landed in my files
 
Reminds me of when my moma fell and fractured her hip..

She got up in the middle of the night to get a drink of water...

Earlier that day she had moved a concrete chicken that was in her flower bed, to the kitchen by the garbage can.

She tripped over it on her way to the sink..

When she got to the hospital they took her statement on a recorder of what happened..

It was written on her report that "apparently she was drinking"

My mother does not and will not drink any kind of alcohol..

Later we had to get it changed, until we did I had to explain to all the doctors that was not the case.

I did it for her because she was so embarrassed by it.

Me..I would have had a huge laugh every time I went....

My 'electronic chart" contains so much erroneous junk in it, it's pretty useless. Both myself and my physician have tried to have things deleted, but apparently once it's in the chat IT'S IN THE CHART FOREVER with the system I have. They are big on talking about providing a great patient/ provider relationship, but so far it's has been just happy talk. Ya know, we don't have appointmentss, "we interface".
