There was an incident 2 days ago at a heavy industry business a few doors from my place of work.
A 16 year old male was using an overhead crane and unfortunately he was standing directly below the suspended load.
Something gave way and the load fell on him. He died of a crush injury.
I use overhead cranes moving loads up to 10 tons. At any moment there could be equipment failure.
With this in mind, we never stand under a suspended load.
A 16 year old does not understand what can go wrong and it was up to his fellow workers to look out for him.
A tragedy that should never have happened.
 

RIP sounds like very sloopy workshop practices? or perhaps apprentices who don't follow instructions?
I had my accident at work thanks and no fault of mine, and I still suffer from the life-changing event.

On the bright side, the Aussie Test batsmen, sorry now have to call them batters, have lost two wickets after just two balls bowled.
 
My wife was working in an office building in San Francisco when a crane on the top of a building under construction across the street became unbalanced. The crane and its load fell 19 stories into the street below, killing the operator, and I believe some others. Under the street where it landed was a parking garage which some of the debris penetrated. Fortunately my wife had taken a bus to work, so we didn’t lose a car in that garage.
