Quick survey for Grandson's assignment

Hello everybody, hope you are staying safe and feeling good.
My grandson is doing a business assignment for school that requires a survey be done of a wide range of age groups.
He asked me to fill it out and share it with as many people as possible.
If you could just take less than 5 minutes to fill it out that would be really appreciated!

That was painless, quick (Not close to 5 minutes, unless you want it to be) , and not invasive, in my opinion.
