Oil and gas industry has the sharpest increase. As did states with a lot of oil & gas business. Over all about 3 deaths per 100,000 employees. Some blame OSHA Occupational Safety Health Administration and others say there are too many outdated regulations.

Knew people who did OSHA investigations and they said most times it came down to ignoring existing safety rules and equipment. That could include the contractor & job site along with individual employees.

Work should be to make a LIVING and not death.

Sometimes corporations demand that workers ignore existing safety rules to cut expense and operating costs and increase their bottom line.

Sadly I've seen it, Usually comes down to a particular supervisor or worksite. To make matters worse when a supervisor onsite directly tells an individual "don't worry" those employees take that as license to ignore even without an order or direct instruction from a boss. Then they box themselves in with productivity numbers because the less time on safety procedures increases their productivity. Their bad habits become common practice. Most bosses won't discipline you for ignoring their order to ignore safety but you wind up getting all the unwanted jobs, assignments or projects. You become blacklisted.

OSHA would have kittens if they saw what goes on in our store. There used to be a department head who was right on top of things. If something was spilled it better be cleaned up instantly. The new clown is so busy micro-managing he doesn't see the leaking pesticides or fertilizer bags. And unfortunately we don't have the proper protection to clean some of it up. So if you see it you'll probably pretend not to. There are days when you hold your breath in some aisles. Poisons will fall down behind shelves. Even if you had the resources to clean it you'd have unscrew the whole shelving system. Some days it burns your eyes, your nose fills up. Yayy big corporate caring about it's worker ants.
