An ear wax spoon, it was used to dig ear wax out of ears.
First thing that comes to mind is hair implement - maybe something the old ladies used to twist a bun in their noggin?

Used for honey? Twirl it to pick the honey up then drip it onto the toast? Saw something similar in a Gran's home as a kid but it was a wooden one.
I have a few other possibilities but since therapy was suggested I'll not bring them up.

Ok how about a fancy nose picker, the holes are bigger.

It reminds me of those handles to put through plastic grocery bags to carry a bunch of them, but platic was scarce in Victorian times so guess it's not for that.

It looks like a pick that comes along with a nut cracker to dig the nuts out of a walnut for example.
