It cost about $15, and I found it in a Signals catalog...a real catalog. LOL I hardly ever get catalogs in the mail anymore, and when I do, they get thrown out, but a Signals catalog? That's a different story!

This book is one of the best things I've ever spent money for...there are tabbed sections to fill in with any and all info that we'd like our loved ones to know after we've, as we say, assumed room temperature:

personal
medical
key contacts
important documents
financial
business
beneficiaries
personal property
insurance
pets
what to pay/close/cancel
email/social media
miscellaneous
my wishes

How I wish I'd had something like this when my mother died, when my son died, when my husband died. We think we know them and discover just how much we didn't know and what bad mind readers we are when we're faced with decisions after they're gone. This book even tells where to find things!

This book isn't just for seniors; it would be a fine thing for any adult to have and complete. Just an FYI...I filled mine out in pencil, LOL...for things that might change like passwords, new accounts and such.

The people you leave behind with thank you many times. Where it asks where to find documents, I mentioned insurance policies, will, things with multiple pieces of paper, but put others that are just single sheets like birth certificate, kids' birth certificates, marriage certificate in the back of the book so they won't have to hunt them.
